Three Levels of Care — From Companionship to Skilled Nursing

Priority Home Care supports families with the right level of help for their situation, whether that means companionship and daily support, hands-on personal care, or licensed nursing services at home.

Understanding the Types of Care We Provide

Companion Care

Support for independent clients who need help with errands, meals, light housekeeping, activity, and companionship.

Personal Care

Hands-on assistance with Activities of Daily Living provided by trained CNAs or PCTs.

Skilled Nursing

Clinical care delivered by licensed nurses for medical needs that require professional oversight.

How Care Begins

Every service begins with a consultation with a care coordinator to discuss the client’s needs and determine the appropriate level of support.

Per Georgia Department of Health regulations, services are initiated with an assessment performed by a licensed Registered Nurse before care begins.

SERVICE LEVEL

Companion Care

Companion care is ideal for individuals who remain largely independent but benefit from support with routine tasks and staying socially engaged.

This level of care does not involve hands-on assistance with Activities of Daily Living.

SERVICE LEVEL

Personal Care Services

Personal care provides hands-on support for individuals who need help with Activities of Daily Living.

These services are delivered by CNAs or PCTs and are appropriate for clients with higher care needs.

• Dressing support
• Grooming and hygiene
• Feeding assistance
• Mobility and transfers
• Toileting support
• Medication reminders
• Very light housekeeping
SERVICE LEVEL

Skilled Nursing

Skilled nursing services are required when clinical care must be delivered by a licensed nurse.

These services are provided by an LPN or RN and support clients with medical needs that require professional oversight.

• Wound care
• Dressing changes
• Trach care
• Medication management
• Health monitoring
• Clinical oversight
• Provider coordination
• Ongoing nursing assessment
